The Registered Nurse (RN) - Home Care is responsible for delivering personalized nursing care to clients within their homes, adhering to care plans and community health standards.
This role includes patient assessment, care plan implementation, education, and support for both patients and families.
Case management and thorough documentation are essential, with a focus on delivering care in line with organizational and community health standards.
Key Responsibilities
Follow a designated care plan aligned with each patient's needs, including medication administration, wound care, and other treatments.
Conduct home visits within assigned geographic areas, providing direct patient care and monitoring.
Observe and document patient conditions, reporting changes to the Clinical or Client Services Manager.
Follow, execute, and document physician orders accurately.
Perform thorough assessments, manage cases, and coordinate patient care effectively.
Record observations, interventions, and evaluations in a timely manner using an electronic documentation system.
Provide compassionate care, including vital signs monitoring, medication administration, wound care, and catheter care as needed.
Assist with personal care tasks, such as feeding, bathing, and ensuring clean bedding.
Manage and order supplies, as necessary.
Maintain clear communication with patients, family members, and other healthcare team members.
Ability to work independently, managing care without direct supervision, while creating rapport with patients and families.
Required Qualifications
Associate’s degree in Nursing.
Minimum of 1 year of Home Care experience and 1 year of Case Management experience.
Reliable transportation for home visits.
Preferred Qualifications
Bachelor’s degree in Nursing.
3+ years of nursing experience in a residential or home care setting.
Excellent communication skills and the ability to develop rapport with patients and their families.
