We are seeking a highly experienced Registered Nurse - Case Management.
In this role, you will build strong relationships with patients, caregivers, and interdisciplinary healthcare teams to understand patient health needs and deliver optimal outcomes.
You will be responsible for coordinating comprehensive care plans, advocating for patient well-being, and managing resources to ensure effective, cost-efficient care, all while minimizing risks to patient health and safety.
Key Responsibilities:
Utilize clinical nursing techniques and case management practices to assess, intervene, and support patients with complex health conditions.
Conduct psychosocial assessments, make psychosocial diagnoses, and plan and implement effective care treatments.
Collaborate with interdisciplinary teams, including social workers, providers, and caregivers, to ensure holistic care.
Provide coaching and mentoring to patients and their families to navigate the healthcare system and make informed health decisions.
Evaluate and adjust care plans as necessary to meet evolving patient needs.
Travel to hospitals, skilled nursing facilities, and patient homes as required to provide on-site case management and support.
Required Qualifications:
2+ years of experience in Case Management, Medical Social Work, or Brief Treatment.
MSW, Medical Social Work, or relevant case management experience.
Preferred Qualifications:
ED experience, Grief and Bereavement, Cultural Responsiveness, and Crisis Intervention experience.
Certification in Geriatric Case Management or similar.
Certifications:
Active, unrestricted RN license in the state.
COVID-19 vaccination and valid driver's license required.
