The Social Worker provides comprehensive case management and discharge planning services to patients in an acute care hospital setting.
This role is based on the medical/surgical floor and focuses on addressing patients' medical and social needs, coordinating resources, and ensuring smooth transitions of care.
The position does not involve behavioral health responsibilities.
Key Responsibilities:
Provide case management services to patients on the medical/surgical floor.
Develop and implement effective discharge plans in collaboration with the healthcare team.
Assist patients and their families in navigating healthcare resources and addressing social determinants of health.
Coordinate post-discharge care, including referrals to community services, rehabilitation facilities, and follow-up care providers.
Communicate with multidisciplinary teams to ensure patient needs are met efficiently.
Participate in a weekend rotation schedule (approximately every fourth weekend).
Required Qualifications:
Active state Licensed Social Worker (LSW) license at the time of submission.
Experience in acute care hospital settings, specifically in case management and discharge planning.
Preferred Qualifications:
Familiarity with medical/surgical unit protocols and practices.
