The Registered Nurse Case Manager plays a vital role in coordinating and managing patient care to ensure optimal health outcomes.
This position requires working closely with patients, families, and healthcare teams to develop individualized treatment plans, facilitate necessary services, and ensure continuity of care.
Key Responsibilities:
Patient Assessment & Care Planning
- Conduct intake assessments with patients and their families to determine care needs.
- Develop and implement individualized treatment plans based on medical, financial, and social factors.
- Adjust treatment plans as necessary to accommodate changing patient conditions.
Care Coordination & Patient Advocacy
- Ensure patients and their families are fully informed, supported, and equipped throughout the care process.
- Coordinate appropriate follow-up services, rehabilitation, and wellness programs.
- Arrange transportation for patients between home and healthcare facilities.
- Advocate for patient needs while working with insurance providers, social workers, and healthcare professionals.
Collaboration & Compliance
- Work closely with multidisciplinary healthcare teams to track patient progress and adjust care strategies.
- Maintain detailed medical records and case documentation.
- Ensure compliance with state regulations and healthcare best practices.
Required Qualifications:
Education:
Bachelor’s degree in Nursing (BSN) or equivalent.
Certifications & Licenses:
Current RN license to practice in thenmmmbbb (required).
Skills & Experience:
- Strong patient care coordination and case management skills.
- Ability to assess, plan, implement, and evaluate patient care effectively.
- Excellent communication and interpersonal skills to work with diverse patient populations.
