1. Deliver patient-centered nursing care in home health settings.
2. Complete comprehensive assessments using OASIS documentation standards.
3. Develop, implement, and evaluate individualized care plans.
4. Collaborate with interdisciplinary teams to coordinate patient care.
5. Monitor and document patient progress using EPIC electronic health record system.
6. Educate patients and families on treatment plans, medications, and health maintenance.
7. Maintain accurate and timely documentation in accordance with regulatory requirements.

1. Active state Registered Nurse (RN) license
2. Basic Life Support (BLS) certification
3. OASIS documentation experience (required)
4. Proficiency with EPIC charting system
5. Ability to work independently in home health environments
6. Strong clinical judgment and time management skills
7. 
   

1. Prior experience with Client systems or patients
2. Experience using Client -issued devices (laptop/work phone)
3. 
   

1. State RN License
2. BLS Certification

Education: Associate's Degree

Certification: Basic Life Support
