We are seeking a compassionate and dedicated Licensed Social Worker to provide support and advocacy for patients and families. This role involves assessing psychosocial needs, developing care plans, coordinating resources, and ensuring compliance with professional standards and healthcare regulations. The ideal candidate will demonstrate empathy, strong communication skills, and the ability to work collaboratively in a multidisciplinary environment.
Key Responsibilities
Conduct psychosocial assessments and develop individualized care plans
Provide counseling and emotional support to patients and families
Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care
Assist patients and families in accessing community resources, financial assistance, and support services
Advocate for patient rights and ensure culturally competent care
Document all patient interactions and maintain accurate records in compliance with healthcare standards
Participate in case conferences, team meetings, and discharge planning
Support crisis intervention and conflict resolution as needed
Required Qualifications
Master’s degree in Social Work (MSW) from an accredited program
Current state licensure as a Licensed Social Worker (LSW or equivalent)
Strong knowledge of social work principles, ethics, and healthcare regulations
Excellent communication, counseling, and problem-solving skills
Ability to work effectively with diverse populations and in multidisciplinary teams
Must have at least 2 years of recent experience in the specialty within the past 2 years.
Preferred Qualifications
Prior experience in healthcare, hospital, or community-based social work settings
Familiarity with electronic health record (EHR) systems
Specialized training in mental health, substance abuse, or case management
Certifications (if any)
Licensed Clinical Social Worker (LCSW) credential (preferred)
Additional certifications in crisis intervention or behavioral health (preferred)
