The Community Health Worker II will support community members by providing outreach, care coordination, and assistance with accessing medical and social services. Key Responsibilities
Engage with community members through outreach and resource navigation.
Build trust and provide education on available health and social service programs.
Coordinate care by connecting individuals to primary care, specialty care, and social service resources.
Conduct Social Determinants of Health (SDOH) screenings and assist with health plan enrollment.
Provide support at multiple work locations, including clinics and community events.
Required Qualifications
High school diploma or equivalent.
1–2 years of experience in community-based health care preferred.
Proficiency in computer operations and internet-based applications.
Bilingual ability preferred.
Certifications
Community Health Worker (CHW) Certification — required.
