Key Responsibilities
- Engage with community members through outreach and resource navigation.
- Build trust and provide education on available health and social service programs.

- Conduct Social Determinants of Health (SDOH) screenings and assist with health plan enrollment.
- Provide support at multiple work locations, including clinics and community events.
Required Qualifications
- High school diploma or equivalent.
- 1–2 years of experience in community-based health care preferred.
- Proficiency in computer operations and internet-based applications.
- Bilingual ability preferred.
Certifications
- Community Health Worker (CHW) Certification — required.
- Certified Enrollment Counselor (CEC) Certification — required.
