The Licensed Practical Nurse (LPN) – Rehab provides basic nursing care under the supervision of a Registered Nurse (RN) or physician in alignment with individualized rehabilitation care plans. The LPN plays a key role in supporting patients through their recovery journey by assisting with daily activities, administering medications, and monitoring patient progress in a rehabilitative setting.
Key Responsibilities
Deliver basic nursing care as directed by an RN or physician
Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting
Monitor and record vital signs, weight, and report on patient progress
Administer oral and injectable medications as prescribed
Perform wound care, dressing changes, and skin assessments
Assist with patient mobility, transfers, and range-of-motion exercises
Observe and promptly report any changes in patient condition to the supervising nurse or physician
Required Qualifications
Current and valid LPN license
CPR/BLS certification
Strong understanding of nursing best practices within a rehabilitation setting
Excellent interpersonal and communication skills
Ability to work effectively as part of a multidisciplinary team
Attention to detail and strong documentation skills
Preferred Qualifications
Previous experience in a rehabilitation or long-term care facility
Familiarity with electronic health record (EHR) systems
