JOB SUMMARY
Provide direct patient care services to residents.
Key Responsibilities
Administer medications and treatments as prescribed by physician, following facility policies and procedures.
Monitor and document patient vital signs, symptoms, and changes in condition.
Assess patients and develop individualized care plans.
Educate patients and their families on health conditions and treatment plans.
Coordinate care with other healthcare professionals, including physicians and therapists.
Maintain accurate and timely patient records.
Respond to emergencies and provide appropriate interventions.
Ensure a clean and safe environment for patients.
Required Qualifications
Current and valid Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the state of practice.
Current Basic Life Support (BLS) certification.
Graduate of an accredited LVN/LPN program.
Knowledge of nursing principles, practices, and procedures.
Ability to work independently and as part of a team.
Strong communication and interpersonal skills.
Proficiency in electronic health record (EHR) systems.
Preferred Qualifications
Previous experience in a long-term care or skilled nursing facility.
Experience with geriatric patients.
Familiarity with wound care.
Proficiency in medication management systems.
Certifications
Basic Life Support (BLS)
