Physical Therapist

JOB SUMMARY
Provide comprehensive physical therapy services to patients in their home environment. Assess, plan, implement, and evaluate individualized treatment programs to improve patient mobility, function, and overall quality of life.

Key Responsibilities
- Conduct thorough patient assessments, including evaluating physical condition, functional limitations, and home safety.
- Develop individualized, evidence-based treatment plans in collaboration with patients, families, and physicians.
- Provide therapeutic interventions to improve strength, range of motion, balance, coordination, gait, and pain management.
- Educate patients and their families on therapeutic exercises, home safety modifications, adaptive equipment, and disease management.
- Document all patient care services accurately and efficiently in accordance with agency policies and regulatory requirements.
- Communicate effectively with physicians, other healthcare professionals, and caregivers to ensure coordinated patient care.
- Maintain professional competence through continuing education and participation in professional development activities.
- Adhere to all ethical and legal standards of practice.

Required Qualifications
- Graduate of an accredited Physical Therapy program.
- Current state of **[State Name]*• Physical Therapist license.
- Basic Life Support (BLS) certification.

Experience
- Minimum of 1 year of experience as a Physical Therapist.
- Prior experience in home health is strongly preferred.

Skills
- Excellent clinical assessment and treatment planning skills.
- Strong understanding of adult learning principles for patient and family education.
- Proficiency in manual therapy techniques, therapeutic exercise, and gait training.
- Effective communication and interpersonal skills.
- Ability to work independently and manage a caseload effectively.
- Knowledge of home safety principles and fall prevention strategies.

EMR / Systems
- Experience with electronic medical record (EMR) systems is required.

Patient Population
- Adults and geriatric patients requiring home-based physical therapy services.

Unit / Specialty
- Home Health

Floating / On-Call
- May be required to cover a designated service area.

Education: Masters Degree
