JOB SUMMARY
Provide comprehensive physical therapy services to patients in their homes, promoting recovery, independence, and improved quality of life. Assess, plan, implement, and evaluate individualized treatment programs.
Key Responsibilities
- Conduct thorough patient evaluations, including assessment of functional status, mobility, strength, range of motion, pain levels, and endurance.
- Develop and implement individualized treatment plans based on patient needs, goals, and physician orders.
- Provide therapeutic interventions such as therapeutic exercise, manual therapy, gait training, balance training, and prosthetic/orthotic training.
- Educate patients and their families on home exercise programs, adaptive equipment, safety precautions, and disease management.
- Monitor patient progress and modify treatment plans as necessary.
- Document all patient interactions, assessments, treatment plans, and progress notes accurately and timely in accordance with agency policies and regulatory requirements.
- Collaborate with the interdisciplinary healthcare team, including physicians, nurses, and other therapists, to ensure coordinated patient care.
- Maintain compliance with all state and federal regulations, licensing requirements, and agency policies and procedures.
- Provide training and mentorship to therapy students or assistants as needed.

Required Qualifications
- Graduate of an accredited Physical Therapy program.
- Current, unrestricted Physical Therapist license in the state of practice.
- Basic Life Support (BLS) certification.
Experience
- Minimum of 1 year of experience as a Physical Therapist.
- Prior experience in home health settings is preferred.
Skills
- Excellent communication, interpersonal, and patient education skills.
- Strong assessment and treatment planning abilities.
- Proficient in documenting within electronic health records.
- Ability to work independently and manage time effectively.
- Knowledge of rehabilitation principles and assistive devices.
