JOB SUMMARY
Provide comprehensive physical therapy services to patients in their homes within San Mateo County, CA. Assess, plan, implement, and evaluate individualized treatment programs to restore function, improve mobility, and promote optimal health and wellness.

Key Responsibilities
- Conduct thorough patient evaluations, including functional assessments and range of motion testing.
- Develop and implement individualized home-based treatment plans in collaboration with patients, families, and physicians.
- Administer therapeutic interventions, including exercise programs, manual therapy techniques, and assistive device training.
- Educate patients and caregivers on home exercise programs, safety, and adaptive techniques.
- Monitor patient progress and adjust treatment plans as needed.
- Document all patient care services accurately and timely in compliance with agency policies and regulatory guidelines.
- Collaborate with interdisciplinary healthcare team members.
- Maintain patient confidentiality and adhere to all ethical and professional standards.

Schedule / Shift
- Full-time position, typically Monday through Friday.
- May require some flexibility to accommodate patient scheduling needs.

Required Qualifications
- Graduation from an accredited physical therapy program.
- Valid Physical Therapist (PT) license in the state of California.
- Current CPR/BLS certification.

Experience
- Minimum of 1 year of experience as a Physical Therapist.
- Experience in home health or community-based care is generally preferred.

Skills
- Strong skills in patient assessment, treatment planning, and intervention.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work independently and manage caseload effectively.
- Proficient in documenting patient care.

EMR / Systems
- Experience with electronic health record (EHR) systems for documentation.

Patient Population
- Adults and geriatric patients requiring home-based rehabilitation services.

Unit / Specialty
- Home Health

Floating / On-Call
- Travel to patient homes within San Mateo County is required.
- May require participation in on-call rotation as per agency policy.
