Job Title: Patient Care Coordinator - Travel

JOB SUMMARY
The Patient Care Coordinator - Travel will ensure that patients and their families are informed of their treatment plan and any necessary appointments or procedures. This role will also coordinate patient care by communicating with physicians, nurses, and other healthcare professionals.

Key Responsibilities
- Facilitate communication between patients, families, and healthcare providers.
- Schedule and manage patient appointments and procedures.
- Provide information on treatment plans and medical services.
- Coordinate with physicians, nurses, and other staff to ensure seamless patient care.
- Document patient interactions and care plans.

Required Qualifications
- High school diploma or GED.
- Previous experience in a healthcare setting.
- Strong communication and interpersonal skills.
- Ability to organize and manage multiple tasks.
- Proficient in electronic health record systems.

Certifications
- Basic Life Support (BLS) certification.
