The Registered Nurse (RN) - Home Health provides skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care that promotes recovery, independence, and overall well-being. The RN is responsible for assessing patient needs, developing and implementing individualized care plans, monitoring patient progress, and collaborating with physicians, caregivers, and interdisciplinary healthcare teams to achieve optimal outcomes.
Key Responsibilities
Conduct comprehensive patient assessments and develop individualized care plans.
Provide skilled nursing services in accordance with physician orders and established care protocols.
Monitor patient health status, evaluate responses to treatment, and revise care plans as needed.
Administer medications, treatments, wound care, and other nursing interventions within scope of practice.
Educate patients and family members on disease management, medications, treatment plans, and preventive care.
Coordinate care with physicians, therapists, social workers, caregivers, and other healthcare professionals.
Maintain accurate, timely, and complete clinical documentation in accordance with regulatory and organizational requirements.
Identify changes in patient condition and communicate findings to appropriate healthcare providers.
Promote patient safety and infection prevention practices in the home setting.
Ensure compliance with federal, state, and agency policies, procedures, and standards.
Participate in quality improvement initiatives and interdisciplinary care meetings.
Perform other duties as assigned.
Required Qualifications
Graduate of an accredited School of Nursing.
Current and unrestricted Registered Nurse (RN) license in the applicable state.
Minimum of one (1) year of recent clinical nursing experience.
Strong clinical assessment, critical thinking, and patient-care skills.
Excellent verbal and written communication abilities.
Ability to work independently and manage patient care in a home environment.
Proficiency in electronic medical records (EMR) and clinical documentation systems.
Valid driver's license, reliable transportation, and ability to travel within the assigned service area.
Preferred Qualifications
Previous Home Health, Hospice, Case Management, or Community Health nursing experience.
Bachelor's Degree in Nursing (BSN).
Experience with OASIS assessments and home health documentation.
Knowledge of chronic disease management and care coordination practices.
Experience working with diverse patient populations in community-based settings.
Certifications
Current Basic Life Support (BLS) certification required.
Home Health or Community Health Nursing certifications preferred, if applicable.
Work Environment
Primary work setting is patient homes within the assigned coverage area.
Frequent local travel required.
May require flexible scheduling, including weekends, holidays, and on-call coverage as needed.
