JOB SUMMARY
Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice. Develop, implement, and carry out individualized treatment plans in collaboration with the interdisciplinary team. Implement evidence-based therapeutic interventions to maximize function, independence, and quality of life. Educate residents, families, and caregivers. Complete accurate, timely, and compliant documentation. Supervise therapy assistants, when applicable to the role. Must be available to work Hawaii State and Federal Holidays as necessary based on facility operations.

Key Responsibilities
• Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).

• Educate residents, families, and caregivers regarding treatment plans and progress.
• Complete accurate, timely, and compliant documentation.
• Supervise therapy assistants, when applicable.
• Work Hawaii State and Federal Holidays as necessary.

Schedule / Shift
• Generally scheduled five (5) days/week.
• Likely to include one weekend day.

Required Qualifications
• Current Basic Life Support (BLS) certification.

Certifications
• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license in good standing appropriate to the discipline.

Experience
• Minimum two (2) years of experience in the SNF/ICF setting preferred.
