JOB SUMMARY
Speech-Language Pathologist (SLP) providing rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).

Key Responsibilities
• Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.
• Develop, implement, and carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team, as appropriate.
• Implement evidence-based therapeutic interventions to maximize function, independence, communication, swallowing, and quality of life.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• Complete accurate, timely, and compliant documentation of progress, changes in the plan of care, and discharges according to facility, state, federal, and payer requirements.
• Supervise therapy assistants, when applicable to role.
• Perform duties permitted under applicable state law and professional practice standards.

Schedule / Shift
• Generally scheduled five (5) days/week, likely to include one weekend day.
• Must be available to work Hawaii State and Federal Holidays as necessary based on facility operations.

Required Qualifications
• Candidate must have a current State of Hawaii license in good standing appropriate to the discipline.
• Any traveler not currently in possession of an active State of Hawaii license must apply upon acceptance of the offer.

Experience
• Minimum two (2) years of experience in the SNF/ICF setting is preferred.

Certifications
• Current Basic Life Support (BLS) certification.

Skills
• Proficient in providing rehabilitation services.
• Knowledge of individualized treatment plans and collaboration with interdisciplinary teams.
• Ability to implement evidence-based therapeutic interventions.
• Effective patient and family education skills.
• Strong clinical documentation skills.
• Supervision of therapy assistants.

EMR / Systems
• Orientation to EMR documentation (Net Health Rehab Optima & Epic) is provided.

Unit / Specialty
• Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).

Floating / On-Call
• Possible floating to other units or facilities based on census and staffing needs.

Education: Doctoral Degree
