We are seeking a skilled and compassionate Physical Therapist (PT) to evaluate, plan, and implement personalized treatment programs that help patients improve mobility, reduce pain, restore function, and enhance overall quality of life. The Physical Therapist will work closely with patients, physicians, and other healthcare professionals to achieve optimal rehabilitation outcomes.
Key Responsibilities
Conduct comprehensive patient evaluations and assessments.
Develop and implement individualized treatment plans based on patient needs and goals.
Administer therapeutic exercises, manual therapy techniques, and other physical therapy interventions.
Monitor patient progress and modify treatment plans as necessary.
Educate patients and their families on treatment plans, injury prevention, home exercise programs, and wellness strategies.
Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care.
Maintain accurate and timely patient documentation in compliance with organizational and regulatory standards.
Ensure a safe and supportive treatment environment.
Participate in discharge planning and recommend appropriate follow-up care.
Stay current with evidence-based practices and advancements in physical therapy.
Required Qualifications
Graduate from an accredited Physical Therapy program.
Active and unrestricted Physical Therapist (PT) license in the applicable state.
Strong knowledge of rehabilitation techniques, therapeutic exercises, and patient care practices.
Excellent communication and interpersonal skills.
Ability to assess patient needs and develop effective treatment plans.
Strong organizational and documentation skills.
Ability to work independently and as part of a multidisciplinary healthcare team.
Preferred Qualifications
Doctor of Physical Therapy (DPT) degree.
Previous experience in acute care, rehabilitation, outpatient, orthopedic, or skilled nursing settings.
Experience treating patients with neurological, orthopedic, sports-related, or post-surgical conditions.
Familiarity with electronic medical record (EMR) systems.
Specialized clinical training in rehabilitation or musculoskeletal therapy.
Certifications
Active Physical Therapist (PT) License.
Basic Life Support (BLS) Certification (preferred).
Specialty certifications such as OCS (Orthopedic Clinical Specialist), NCS (Neurologic Clinical Specialist), or other relevant certifications are a plus.
