Conducting evaluations to determine patient eligibility for cardiac rehabilitation programs
Evaluating patients’ risk factors for heart disease and stroke, such as blood pressure, cholesterol levels, family history, smoking status, diet, exercise level, etc.
Creating an individualized treatment plan for each patient based on their needs, goals, and preferences
Performing diagnostic testing to assess the patient’s cardiovascular status, including electrocardiograms (EKGs) and stress tests
Supervising exercise regimens to ensure that patients are adhering to their exercise plans
Providing education about proper diet and nutrition to patients with heart issues
Coordinating care with physicians, nurses, and other healthcare professionals involved in the patient’s treatment plan
Communicating with patients and their families to discuss progress in treatment plans
Creating follow-up plans with patients to ensure they continue following their treatment plans after leaving the facility
