As a Home Health Registered Nurse (RN), you will play a vital role in assessing patient conditions, implementing individualized care plans, and providing continuous education and support to patients and their families. You will deliver patient care in alignment with the organization’s philosophy, policies, and community health practice standards. Your responsibilities will also include performing admission processes and managing cases in accordance with MGB Home Care’s policies and standards, ensuring thorough and timely electronic documentation and effective interdisciplinary communication.
Key Responsibilities:
Conduct comprehensive patient assessments to determine healthcare needs.
Develop, implement, and monitor individualized care plans.
Provide direct patient care in accordance with clinical guidelines and best practices.
Offer education and support to patients and their families regarding health conditions and care management.
Perform patient admissions and manage case load effectively.
Maintain accurate and timely electronic health records.
Collaborate with interdisciplinary teams to ensure cohesive patient care.
Participate in weekend rotations (once every 6 weeks) as required.
Travel to assigned cases as needed.
Required Qualifications:
Minimum of 1 year of Certified Home Care experience.
At least 1 year of Case Management experience.
Ability to travel to assigned cases.
Strong clinical assessment and decision-making skills.
Excellent communication and interpersonal abilities.
Preferred Qualifications:
Experience with electronic medical records (EMR) systems.
Previous experience in a home health setting.
Certifications:
Current and unrestricted Registered Nurse (RN) license.
Basic Life Support (BLS) certification (required).
Advanced Cardiovascular Life Support (ACLS) certification (preferred).
