We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality, patient-centered care in a home health setting. In this role, you will assess patient health, develop and implement effective care plans, and collaborate with physicians and family members to support patients' recovery and well-being. Your expertise will be vital in delivering care that promotes health, prevents illness, and enhances the quality of life for our patients.
Key Responsibilities:
Patient Assessment & Monitoring:
Evaluate and monitor patients’ progress, identifying sudden or subtle changes in their medical condition.
Conduct thorough assessments to develop and adjust individualized care plans.
Care Delivery:
Administer treatments, monitor medication doses, and ensure proper implementation of care plans.
Respond promptly to medical emergencies, alerting appropriate healthcare providers as needed.
Provide care for patients throughout their recovery, including post-ICU care.
Collaboration & Communication:
Deliver regular updates on patient status to doctors, patients, and family members.
Coordinate with interdisciplinary teams to ensure comprehensive care management.
Documentation & Compliance:
Maintain accurate and up-to-date patient records, documenting care provided and patient outcomes.
Complete all required paperwork prior to patient transfers.
Required Qualifications:
Education: Associate of Science in Nursing (ASN) or higher.
Experience: Minimum of 2 years of nursing experience, preferably in a home health or critical care setting.
Licensure & Certifications:
Valid Registered Nurse (RN) License.
Basic Life Support (BLS) certification.
Proof of COVID-19 vaccination (as per organizational policy).
Preferred Qualifications:
Strong clinical assessment and critical thinking skills.
Ability to work independently in a home health setting.
Excellent communication and interpersonal skills to engage effectively with patients, families, and healthcare teams.
Proficiency in electronic health records (EHR) and medical documentation.
