Licensed Clinical Social Worker

Job Summary:
The Social Worker is a vital member of the interdisciplinary team, providing and overseeing psychosocial care for selected patients and families. The role involves conducting psychosocial assessments, delivering intervention and treatment, and identifying high-risk psychosocial factors that impact health outcomes. The Social Worker plays a key role in helping hospital staff understand how these factors influence the course of medical care. The position requires working effectively with an interdisciplinary health care team, addressing psychosocial, environmental, and cultural issues, and maximizing emotional, social, and physical well-being for patients.

Key Responsibilities:
  • Conduct psychosocial assessments of patients and families, identifying high-risk psychosocial factors that may impact health status.
  • Provide intervention and treatment as indicated, addressing complex psychosocial issues such as homelessness, protective services, frailty, disability, psychiatric and substance abuse concerns.
  • Work collaboratively as part of an interdisciplinary health care team to ensure comprehensive care.
  • Provide clinical services that address psychosocial, environmental, age-specific, and cultural factors to optimize the effective use of healthcare and community resources.
  • Assist hospital staff in understanding the psychosocial influences on patients’ health and treatment plans.
  • Coordinate with other healthcare providers and community services to maximize patients' emotional, social, and physical well-being.
  • Operate within established organizational and departmental policies and procedures, reporting to the Nurse Manager/Nurse Director.

Required Qualifications:
  • Master’s degree in Social Work from an accredited school of social work, or an equivalent degree with independent licensure required.
  • Licensure as a Certified Social Worker (LICSW, LCSW, LMHC) in the state.
  • Minimum of three (3) to five (5) years of experience in medical social work within an acute care hospital setting.
  • Proficiency with personal computers and ability to learn departmental software.
  • Strong interpersonal, oral, and written communication skills, with demonstrated cultural awareness.
Preferred Qualifications:
  • Experience in behavioral health and substance abuse is preferred.
  • Strong knowledge of healthcare services across the continuum.

Certifications:
  • Licensure as a Certified Social Worker (LICSW, LCSW, LMHC) in the State required.
