1. Implement and follow individualized care plans tailored to each patient’s needs
2. Conduct home visits within assigned geographic areas
3. Administer medications, perform wound care, and carry out other prescribed treatments and procedures
4. Monitor patient conditions and report significant changes to the Clinical Manager or Client Services Manager
5. Accurately execute and document physician orders
6. Perform comprehensive patient assessments and case management when necessary
7. Maintain detailed, real-time documentation in electronic health record (EHR) systems
8. Regularly take and track vital signs
9. Ensure accurate medication administration and tracking
10. Provide catheter care, assist with feeding, bathing, hygiene, and other activities of daily living (ADLs)
11. Maintain clean and sanitary living conditions for patients
12. Track and order medical supplies as needed
13. Demonstrate empathy, professionalism, and reliability in patient interactions
14. Adapt to various shift schedules and patient needs
15. Drive to and from patient residences with reliable transportation
16. Build strong rapport with patients and family members
17. Work independently with confidence in providing high-quality care without direct supervision

1. Associate’s Degree in Nursing (ADN) – Required
2. Bachelor of Science in Nursing (BSN) – Preferred

1. Minimum 3 years of nursing experience in a residential, home health, or similar setting

1. Experience with electronic medical record (EMR/EHR) systems
2. Background in community health or mobile/field nursing

1. Active RN license in the state of practice – Required
2. BLS certification – Required
3. ACLS certification – Preferred
