The Licensed Nurse Care Coordinator is responsible for overseeing patient care coordination across healthcare services. This role ensures that patients receive comprehensive, continuous, and compassionate care by collaborating with physicians, nurses, and other healthcare professionals. The Care Coordinator serves as a key liaison between patients, families, and the medical team to promote quality outcomes and patient satisfaction.
Key Responsibilities
Coordinate patient care plans across multiple providers and healthcare settings.
Conduct patient assessments and identify care needs.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on treatment plans, medications, and follow-up care.
Facilitate communication between patients, families, and interdisciplinary teams.
Ensure compliance with healthcare regulations, facility policies, and documentation standards.
Support transitions of care, including hospital discharge planning and community resource referrals.
Required Qualifications
Current Licensed Nurse (LPN or RN) license in the state of practice.
Minimum of 2 years of clinical nursing experience.
Strong organizational and communication skills.
Ability to work collaboratively with interdisciplinary teams.
Knowledge of care coordination processes and patient advocacy.
Preferred Qualifications (if any)
