We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will provide emotional support, counseling, and resource coordination to patients and families, ensuring they receive the assistance needed to cope with medical, social, and emotional challenges.
Key Responsibilities
Assess patients’ psychosocial needs and develop individualized care plans
Provide counseling and emotional support to patients and families
Coordinate community resources, referrals, and support services
Collaborate with healthcare providers, nurses, and interdisciplinary teams to ensure holistic patient care
Advocate for patients’ rights and assist with navigating healthcare systems
Document assessments, interventions, and outcomes in compliance with regulatory standards
Support crisis intervention and conflict resolution when necessary
Educate patients and families on available programs, benefits, and coping strategies
Required Qualifications
Bachelor’s degree in Social Work (BSW) or related field
Must have at least 2 years of recent experience in the specialty within the past 2 years.
Strong communication, counseling, and interpersonal skills
Ability to work independently and collaboratively within a multidisciplinary team
Preferred Qualifications (if any)
Master’s degree in Social Work (MSW)
Prior experience in hospital or clinical social work
Knowledge of community resources and healthcare systems
Certifications (if any)
State licensure or certification as a Social Worker (as required by local regulations)
