Comprehensive Perinatal Health Worker

Job Summary
Client  is seeking a full-time Comprehensive Perinatal Health Worker (CPHW) to provide frontline support to pregnant and postpartum patients through the Comprehensive Perinatal Services Program (CPSP). This role focuses on case coordination, patient advocacy, education, and outreach to ensure patients receive comprehensive, high-quality care during the prenatal and postpartum periods.

Key Responsibilities
  1. Case Coordination: Provide prenatal and postpartum case management, including assessments, care plan development, and follow-up.
  2. CPSP Compliance: Ensure program compliance with CPSP guidelines, including billing, coding, and auditing of service units.
  3. Patient Advocacy & Education: Educate patients on prenatal care, nutrition, lactation support, and psychosocial needs.
  4. Outreach & Referrals: Conduct outreach to families, provide internal and external referrals, and act as a liaison between patients and medical providers.
  5. Documentation: Maintain accurate, confidential records, update databases, and compile monthly reports.

Required Qualifications
  1. Family Planning Health Worker Certificate or CPHW certification (training may be arranged by Client  within 6 months of hire if not already certified).
  2. Previous experience in community health, patient navigation, or maternal health.
  3. Strong communication skills, empathy, and ability to work with diverse populations.

Preferred Qualifications (if any)
  1. Experience working in perinatal or maternal health programs.
  2. Familiarity with CPSP guidelines and compliance requirements.

Certifications (if any)
  1. Family Planning Health Worker Certificate or CPHW certification required (or obtained within 6 months of hire).

Education: Associate's Degree
