Job Summary
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide in-home healthcare services. The ideal candidate will have strong clinical skills, attention to detail, and the ability to deliver high-quality care in a home health setting. This role requires experience with Epic Homecare, strong communication abilities, and a proactive approach to patient care.
Key Responsibilities
- Evaluate and monitor patients’ progress in the home setting
- Identify and respond to sudden or subtle changes in a patient’s medical condition
- Communicate updates clearly to physicians, patients, and family members
- Begin treatment and monitor medication dosages
- Respond to medical emergencies and notify appropriate healthcare providers
- Complete all necessary documentation prior to patient transfers
- Maintain up-to-date and accurate patient records
- Develop and implement effective, individualized care plans
Required Qualifications
- Education: Bachelor of Science in Nursing (BSN)
- Experience: Minimum 2 years of current LPN experience, specifically in Med Surg unit
- Technical Skills: Experience with Epic Homecare system
- Licensure: Active LPN License
Certifications:
- Basic Life Support (BLS) – Required
- COVID-19 vaccination – Required
Preferred Qualifications
- Strong interpersonal and communication skills
- Ability to work independently in a home-based care environment
- Experience handling medical emergencies in a non-hospital setting
- Organizational skills and attention to documentation accuracy
Certifications
- LPN License (if required for position)
- BLS Certification – Required
