JOB SUMMARY
Provides basic nursing care to patients in a long-term care setting under the supervision of a Registered Nurse.

Key Responsibilities
- Administer medications and treatments.
- Monitor patient vital signs and conditions.
- Document patient observations and care provided.
- Assist with patient hygiene and daily living activities.
- Communicate with patients, families, and healthcare team members.

Schedule / Shift
Evenings Shift: 2:30 PM - 10:30 PM
Every other weekend required.

Required Qualifications
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in New York.
- 1+ years of nursing experience.
- BLS certification.

Experience
- Experience in long-term care.
- Experience with MS, ortho, dementia, and memory care patient populations.

Skills
- Proficient in administering medications and treatments.
- Skill in patient assessment and monitoring.
- Ability to document accurately and comprehensively.
- Strong communication and interpersonal skills.

Floating / On-Call
- Must float to all units within the same building.

Education: Certification
