JOB SUMMARY
Provides skilled physical therapy services in a patient’s home environment under the direction of a physician, in accordance with physician’s orders, and as directed by the agency's policies and procedures.

Key Responsibilities
• Performs patient assessments, develops and implements individualized treatment plans.
• Provides therapeutic exercises, manual therapy, and modalities to improve patient function.
• Educates patients and caregivers on exercises, assistive devices, and safety precautions.
• Documents patient progress and treatment provided accurately and timely.
• Collaborates with the healthcare team to ensure continuity of care.

Schedule / Shift
• Every other weekend required.

Required Qualifications
• Valid PTA license in the state of practice.
• Minimum of 1 year of PTA experience in Home Health.

Certifications
• Basic Life Support (BLS) certification.

Experience
• Home Health experience is required.
• Accepts first-time travelers with Home Health experience.

Skills
• Proficient in developing and implementing physical therapy treatment plans.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Patient Population
• Diagnoses include but not limited to: Joint Replacement/Ortho Aftercare, CVA, OA, DM, HTN, CHF, COPD, CKD, Dementia, Aftercare, Amputation, Surgical/Non Surgical Wounds, Cancer, UTI & PVD.

Floating / On-Call
• Floating within scope of practice.
• No on-call duties.

HOLIDAYS
• Travelers are expected to work holidays that fall within their assignment period based on the following: one major holiday requires working, two major holidays require working both, and three major holidays require working two of the three. Major holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Years Eve/New Years Day.
