JOB SUMMARY
Provide comprehensive physical therapy services to patients in their homes to address physical impairments, functional limitations, and disabilities resulting from various causes. Collaborate with patients, families, physicians, and allied health professionals to develop and implement individualized care plans. Ensure accurate and timely electronic documentation and communication according to agency policies and professional standards.
Key Responsibilities
• Assess patients' functional status, identify impairments, and establish measurable goals.
• Develop and implement individualized treatment plans utilizing therapeutic exercises, modalities, and adaptive equipment.
• Educate patients and families on home exercise programs, safe transfers, and adaptive strategies.
• Monitor patient progress and modify treatment plans as needed.
• Document all treatments, progress notes, and communications in the electronic medical record.
• Coordinate care with physicians, other healthcare professionals, and community resources.
• Advocate for patients' needs and promote independence and quality of life.
• Maintain professional competency and adhere to ethical standards of practice.
Schedule / Shift
Weekend rotations are required once every 6 weeks.
Required Qualifications
• Current Physical Therapist professional license in Massachusetts.
• Demonstrated ability to travel to patient cases as assigned.
Experience
• 1 year of Home Care experience.
• 1 year of Case Management experience within a certified home health agency.
Skills
• Proficiency in providing physical therapy interventions for a wide range of diagnoses.
• Strong assessment, evaluation, and treatment planning skills.
• Excellent communication, interpersonal, and motivational skills.
• Ability to work independently and manage time effectively.
• Competent in electronic documentation and charting.
EMR / Systems
• Experience with electronic documentation in accordance with agency policies.
Patient Population
• Patients may range in age from birth to over 100 years, residing in their place of residence.
Unit / Specialty
• Home Health
Floating / On-Call
• Ability to travel to cases as assigned.
• Weekend rotations are required once every 6 weeks.
