JOB SUMMARY
Provide comprehensive physical therapy evaluations and treatments across various settings, including inpatient (acute care, ICU, SNF) and outpatient services. This role requires an independent and thorough approach to managing a diverse patient caseload, emphasizing mobility progression, discharge planning, and the ability to adapt care based on patient needs and diagnoses.

Key Responsibilities
• Perform thorough physical therapy evaluations to assess patient conditions and develop individualized treatment plans.
• Administer therapeutic interventions and treatments to improve patient mobility, strength, balance, and overall function.
• Manage a mixed caseload of inpatient and outpatient patients, providing effective care in acute, ICU, SNF, and outpatient clinic settings.
• Collaborate with interdisciplinary healthcare teams to facilitate patient progress and coordinate care.
• Develop and implement effective discharge plans, ensuring continuity of care and patient/family education.
• Adapt treatment approaches for various diagnoses across all patient populations served.
• Provide care in emergency room, intensive care unit, medical-surgical units (acute, observation, and skilled nursing facilities), obstetrics, and the hospital-based outpatient physical therapy clinic.
• Document patient progress and treatment plans accurately and comprehensively.

Schedule / Shift
• Full PT schedule involves a mix of inpatient and outpatient patients scheduled every 30-45 minutes, with new evaluations lasting up to one hour.
• Estimated patient load: 5 to 15 patients per day.

Required Qualifications
• Graduation from an accredited Physical Therapy program.
• Current Physical Therapist license in Hawaii.

Certifications
• Basic Life Support (BLS) certification may be required.

Experience
• Demonstrated experience in acute care, ICU, SNF, and outpatient physical therapy settings.
• Experience with discharge planning and case management.
• Proven ability to work independently and manage a varied patient load.

Skills
• Proficiency in evaluating and treating a wide range of orthopedic and neurological conditions.
• Strong understanding of mobility progression and rehabilitation principles.
• Excellent communication and interpersonal skills.
• Ability to collaborate effectively within a healthcare team.
• Skilled in discharge planning and patient education.

EMR / Systems
• Experience with electronic health record (EHR) systems for documentation.

• Willingness to float between inpatient units (Acute, ICU, SNF) and the outpatient clinic as needed.
• Potential for on-call duties may apply based on facility needs.
