JOB SUMMARY
Licensed Practical Nurse (LPN) to provide direct patient care in a Long Term Care (LTC) facility located on the hospital campus. Responsibilities include assessments, treatments, medication administration, and assisting patients with daily living activities.

Key Responsibilities
• Perform patient assessments and treatments.
• Administer medications.
• Notify physician or Nurse Practitioner of pertinent patient information.
• Manage patient admissions and discharges.
• Provide assistance with Activities of Daily Living (ADL).
• Perform Gastrostomy Tube care.
• Perform Urinary Catheter care.
• Administer Nebulized Treatments.
• Provide Wound Vac care.
• Manage CPAP care.
• Perform Tracheostomy Care.
• Perform other assigned duties.

Schedule / Shift
• 8-hour shifts.
• Varied shifts, including weekends and holidays.
• Candidate must be flexible. No guarantee of hours or shift preference.

Required Qualifications
• Must possess a valid Licensed Practical Nurse (LPN) license.
• Proof of flu vaccination required prior to contract start date. Flu vaccination can be administered during employee health clearance appointment.

Certifications
• Basic Life Support (BLS) certification is typically required.

Experience
• Experience in a Long Term Care (LTC) setting is preferred.

Skills
• Proficiency in medication administration.
• Skilled in performing various patient treatments and procedures.
• Ability to conduct patient assessments.
• Competent in providing ADL assistance.
• Experience with Gastrostomy Tube care, Urinary Catheter care, Nebulized Treatments, Wound Vac care, CPAP care, and Tracheostomy Care.

EMR / Systems
• Experience with EMR/EHR systems is generally expected.

Patient Population
• Adult patients requiring long-term care.

Floating / On-Call
• Floating expectations may apply within the facility.

Education: Certification
