JOB SUMMARY
Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes. Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.

Key Responsibilities
• Deliver comprehensive physical therapy evaluations and treatments to patients in their homes.
• Develop and implement individualized treatment plans based on patient assessments.
• Educate patients and their families on home exercise programs and adaptive strategies.
• Communicate effectively with physicians, interdisciplinary team members, and patient families.
• Maintain accurate and timely patient records using electronic documentation systems.
• Adapt to a variety of patient needs and functional levels.
• Travel to patient residences as assigned.

Schedule / Shift
• Weekend rotations are once every 6 weeks.

Required Qualifications
• Physical Therapist Professional license in Massachusetts.
• 1 year of Home Care experience required.
• 1 year of Certified Home Health Care Management experience.
• Experience in hospice or private care does not meet this qualification.
• Experience as a Case Manager on its own does not meet this qualification.
• Only roles specifically within a certified home health agency will be considered relevant for this requirement.
• Ability to travel to cases as assigned.

Experience
• 1 year Home Care experience.
• 1 year Case Management Experience required within a certified home health agency.

Skills
• Proficient in providing home-based physical therapy.
• Strong patient education and family communication skills.
• Ability to develop and implement effective treatment plans.
• Proficiency in electronic documentation.

EMR / Systems
• Utilize electronic documentation in accordance with agency policies and standards.

Patient Population
• Patients may range in age from birth to over 100 years.

Floating / On-Call
