JOB SUMMARY
Seeking a dedicated Physical Therapist to provide services for an Early Intervention Program supporting children from birth to three years old. This role involves working in the child's natural environment, collaborating with families and childcare providers, and focusing on individual development goals.

Key Responsibilities
• Provide direct physical therapy services to children aged birth to three.
• Work collaboratively with families and childcare providers in the community.
• Develop and implement Individualized Family Service Plans (IFSPs).
• Bill Medicaid for services rendered.
• Make service recommendations, develop goals, and monitor child progress.
• Conduct evaluations, with an average of 1-3 evaluations per day, at least one day per week.
• Manage an average caseload of 50+ children, which may fluctuate seasonally.
• Provide services primarily in the child's natural environment (e.g., home-based).

Schedule / Shift
• Monday through Friday.
• Expected hours: 40 hours per week.
• Candidate will only be paid for hours worked. No pay for school holidays or closings.

Required Qualifications
• Licensed Physical Therapist (PT) in the state of Oregon.

Experience
• Experience with IFSPs.
• Experience billing Medicaid.
• Experience working with the birth to three population is a plus, but new graduates are welcome.

Skills
• Understanding of IFSP development and implementation.
• Proficiency in service recommendation, goal development, and progress monitoring.
• Excellent communication and interpersonal skills for working with families and providers.
• Ability to conduct comprehensive evaluations.

Certifications
• Physical Therapist (PT) License - Oregon

Patient Population
• Birth to three years old.
