JOB TITLE: Certified Occupational Therapy Assistant (COTA) - Long Term Care

JOB SUMMARY
Provide rehabilitation services to residents in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF) setting. Develop and implement individualized treatment plans to maximize resident function, independence, and quality of life, adhering to professional practice standards and state regulations.

Key Responsibilities
• Perform evaluations, reassessments, and administer treatments consistent with licensure and scope of practice.
• Develop and implement individualized treatment plans in collaboration with the interdisciplinary team, physicians, and supervising therapists.
• Utilize evidence-based interventions to improve mobility, communication, swallowing, and overall functional independence.
• Educate residents, families, and caregivers on treatment plans and progress.
• Complete accurate, timely, and compliant documentation of patient progress, changes in the plan of care, and discharges.
• Supervise Therapy Assistants as applicable.
• Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).
• Perform duties permitted under applicable state law and professional practice standards.

Schedule / Shift
• Typically scheduled five (5) days per week, likely including one weekend day.
• Must be available to work Hawaii State and Federal Holidays as necessary.

Required Qualifications
• Current Basic Life Support (BLS) certification.
• Current State of Hawaii license appropriate to the discipline.
• Proficiency in rehabilitation services within a Skilled Nursing Facility (SNF) or Intermediate Care Facility (ICF) setting.
• Ability to adhere to applicable state laws and professional practice standards.

Experience
• Minimum of two (2) years of experience in the SNF/ICF setting preferred.

Skills
• Therapeutic intervention and treatment planning.
• Patient and family education.
• Accurate and timely clinical documentation.
• Supervision of Therapy Assistants (if applicable).

EMR / Systems
• Orientation to Net Health Rehab Optima and Epic will be provided.

Unit / Specialty
• Skilled Nursing Facility (SNF) / Intermediate Care Facility (ICF) / Long Term Care (LTC)

Floating / On-Call
• May be required to float to other units or facilities based on census and staffing needs.
