JOB SUMMARY
The Physical Therapy Assistant (PTA) will provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Responsibilities include performing evaluations, implementing treatment plans, educating patients and families, and completing documentation.
Key Responsibilities
- Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF).
- Perform evaluations, reassessments, treatments, and other clinical services consistent with licensure, discipline, and scope of practice.
- Develop, implement, or carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team.
- Implement evidence-based therapeutic interventions to maximize function, independence, mobility, and quality of life.
- Educate residents, families, and caregivers regarding treatment plans and patient progress.
- Complete accurate, timely, and compliant documentation of progress, changes in the plan of care, and discharges.
- Supervise therapy assistants, when applicable to the role.
- Be available to work Hawaii State and Federal Holidays as necessary.
Schedule / Shift
- Generally scheduled five (5) days/week.
- Likely to include one weekend day.
Required Qualifications
- Current Basic Life Support (BLS) certification.
- Current State of Hawaii license appropriate to the discipline in good standing.
- Any traveler not currently in possession of an active State of HI license must apply upon acceptance of offer. Candidate must have Hawaii License by start of assignment.
Experience
- Minimum two (2) years of experience in the SNF/ICF setting preferred.
EMR / Systems
- Orientation to Net Health Rehab Optima and Epic EMR documentation provided.

Floating / On-Call
- Possible floating to other units or facilities based on census and staffing needs.
