JOB SUMMARY
The Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) provides direct patient care to critically ill patients requiring advanced monitoring and interventions. This role involves assessing patient conditions, administering medications, and implementing treatment plans in accordance with established protocols. The LPN/LVN will utilize advanced monitoring devices, interpret data, and intervene in cardiopulmonary emergencies. Additionally, the role requires educating patients and their families, promoting advocacy, and collaborating effectively within the healthcare team.
Key Responsibilities
• Monitor patients requiring the use of monitoring devices, including heart rates, breathing rates, blood pressure, and oxygen levels.
• Provide bedside patient monitoring and administer medications and other support.
• Perform remote patient monitoring from a central nursing station as applicable.
• Demonstrate competency in basic and advanced life support.
• Apply dysrhythmia monitoring techniques, interpretation, and treatment.
• Perform medication dosage calculations and monitor patients for medication effects.
• Administer continuous medication infusions, including non-titrated vasoactive agents, anti-arrhythmic agents, platelet inhibitors, and/or insulin.
• Monitor patients using standardized procedures for pre-, intra-, and post-procedures such as TEE, cardiac catheterization with PCI, cardioversion, bronchoscopy, EGD, PEG placement, and chest tube insertion.
• Recognize symptoms of cardiopulmonary emergencies and initiate interventions to stabilize patients awaiting transfer to critical care.
• Perform hemodynamic monitoring, including equipment setup and troubleshooting.
• Monitor patients for signs and symptoms of instability and interpret findings.
• Interpret ABGs and communicate findings.
• Assess ventilated patients to ensure prescribed treatment delivery and proper patient response.
• Recognize indications for and manage patients requiring non-invasive oxygen delivery systems, including oral airways, BiPAP, and nasal CPAP.
• Assess and understand patients on long-term mechanical ventilation and the weaning process.
• Assess, monitor, and manage patients requiring renal therapeutic interventions such as hemodialysis, peritoneal dialysis, continuous bladder irrigation, stents, and urostomies.
• Recognize indications for and complications of enteral and parenteral nutrition.
• Assess and address family needs to facilitate transition from hospital to home.
• Demonstrate advocacy, clinical judgment, caring practices, systems thinking, response to diversity, collaboration, and clinical inquiry.
• Educate patients and their families on their care.
Required Qualifications
• Valid California LPN/LVN License.
• Basic Life Support (BLS) certification through the American Heart Association.
• IV Certification.
Experience
• Demonstrated experience working with critically ill patients.
• Experience with hemodynamic monitoring, dysrhythmia interpretation, and advanced life support interventions.
• Experience in patient and family education.
• Must have recently completed orientation at a Kindred facility.
Skills
• Advanced assessment skills.
• Proficiency in medication administration and monitoring.
• Ability to interpret diagnostic data (e.g., ABGs, hemodynamic parameters).
• Competent in managing patients on mechanical ventilation and renal replacement therapies.
• Strong communication and interpersonal skills.
• Critical thinking and problem-solving abilities.
• Ability to collaborate effectively with a multidisciplinary team.
EMR / Systems
• Familiarity with electronic medical record systems in a hospital setting.
