JOB SUMMARY
A Physical Therapist is needed to provide comprehensive therapy services to patients at a rehabilitation facility.
Key Responsibilities
• Assess patient's physical condition, functional mobility, and potential for rehabilitation.
• Develop and implement individualized treatment plans to improve patient's strength, range of motion, balance, coordination, and functional independence.
• Educate patients and their families on treatment plans, home exercise programs, and adaptive equipment.
• Document all patient assessments, interventions, and progress according to facility policies and regulatory standards.
• Collaborate with interdisciplinary healthcare team members to ensure coordinated patient care.
• Maintain patient records and manage caseload efficiently.
Schedule / Shift
36 hours guaranteed. This role is expected to cover spring and summer seasons.
Required Qualifications
• Graduate of an accredited Physical Therapy program.
• Current state license as a Physical Therapist.
Experience
• Experience in a skilled nursing or rehabilitation setting is preferred.
Certifications
• Basic Life Support (BLS) certification, if required by facility.
Skills
• Strong understanding of anatomy, physiology, and kinesiology.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work independently and as part of a team.
