The Licensed Practical Nurse (LPN/LVN) provides clinical and administrative support within an ambulatory care setting, ensuring high-quality patient care and effective communication between patients, providers, and healthcare teams. This role is heavily focused on managing telephone and electronic patient communications, documenting patient concerns, utilizing EPIC EMR, and escalating clinical issues appropriately within the scope of practice.
The ideal candidate will have prior ambulatory or clinic experience, strong communication skills, proficiency with EPIC EMR, and the ability to manage patient inquiries through telephone and electronic messaging systems while maintaining compliance with healthcare regulations and organizational standards.
Key Responsibilities
Patient Care & Clinical Support
Perform patient care activities within the scope of LPN/LVN practice and applicable regulations.
Assess and address patient concerns through telephone and electronic communication channels.
Document patient interactions accurately in the Electronic Medical Record (EMR).
Report unusual findings and patient concerns to the Registered Nurse or appropriate healthcare provider.
Contribute recommendations and observations to support patient care planning.
Ensure care is delivered in accordance with age-specific and individualized patient needs.
Telephone & Electronic Message Management
Manage incoming telephone calls, patient portal messages, and email communications within the scope of practice.
Utilize department-approved tools, including SmartPhrases and escalation protocols.
Triage and route clinical and non-clinical inquiries appropriately.
Escalate urgent or complex issues according to established procedures.
Provide timely and professional responses while maintaining patient confidentiality.
Communication & Collaboration
Utilize effective communication strategies to achieve positive patient and operational outcomes.
Follow established chain-of-command procedures and escalation pathways.
Collaborate with providers, nurses, and other departments to resolve patient concerns.
Support coordinated care and continuity of services across departments.

Participate in problem identification and resolution efforts.
Support process improvements and contribute new ideas when appropriate.

Required Qualifications
Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the applicable state.
Previous ambulatory care, outpatient clinic, or physician practice experience.
Experience using EPIC Electronic Medical Record (EMR).
Strong verbal and written communication skills.
Ability to manage telephone and electronic patient communications effectively.
Knowledge of nursing practices, patient care, and clinical documentation.
Strong organizational, multitasking, and problem-solving abilities.
Ability to work effectively in a team-oriented healthcare environment.
Preferred Qualifications
Telephone triage or telephone messaging experience.
Experience working in a high-volume ambulatory or outpatient setting.

Certifications (If Any)
Current LPN/LVN License (Required)
Basic Life Support (BLS) Certification (Preferred or Required based on facility policy)
