Registered Nurse - Clinical Care Leader

Job Summary
The RN Clinical Care Leader (CCL) is responsible for utilizing the nursing process, including assessment, diagnosis, planning, implementation, and evaluation, to deliver individualized patient care within the home care or residential care setting. This role collaborates with interdisciplinary healthcare teams to coordinate care, promote positive patient outcomes, and ensure compliance with professional nursing standards and regulatory requirements.

The Clinical Care Leader serves as a clinical resource and leader, providing oversight of resident care activities, supervising unlicensed personnel, supporting care coordination efforts, and utilizing critical thinking and clinical judgment to address complex patient needs. This position requires strong communication, leadership, and care management skills to effectively support residents, families, and healthcare team members.

Key Responsibilities
  1. Patient Care & Clinical Assessment
  2. Utilize the nursing process to assess, plan, implement, and evaluate individualized patient care.
  3. Conduct comprehensive resident assessments and monitor changes in condition.
  4. Develop, update, and coordinate patient care plans based on clinical findings and patient needs.
  5. Provide age-appropriate and population-specific nursing care in accordance with established standards of practice.
  6. Perform nursing interventions, patient education, triage, and care coordination activities.
  7. Care Coordination & Collaboration
  8. Collaborate with physicians, therapists, social workers, and other interdisciplinary team members to ensure continuity of care.
  9. Coordinate services and resources to support optimal patient outcomes.
  10. Facilitate communication between residents, families, caregivers, and healthcare providers.
  11. Participate in care conferences and interdisciplinary planning meetings.
  12. Leadership & Supervision
  13. Provide clinical leadership and guidance to nursing staff and unlicensed personnel.
  14. Delegate responsibilities appropriately while maintaining accountability for assigned patient care.
  15. Monitor and supervise care provided by support staff.
  16. Serve as a clinical resource and mentor for team members.
  17. Participate in departmental initiatives, task forces, committees, and organizational projects.
  18. Documentation & Regulatory Compliance
  19. Document patient histories, assessments, symptoms, medications, treatments, and outcomes accurately and timely.
  20. Maintain compliance with state, federal, and organizational documentation requirements.
  21. Ensure nursing practices align with regulatory standards, policies, and procedures.
  22. Support quality improvement, patient safety, and compliance initiatives.
  23. Technology & Clinical Systems
  24. Utilize electronic health record (EHR/EMR) systems and other healthcare technologies to support patient care and communication.
  25. Leverage technology to enhance care coordination, documentation accuracy, and clinical decision-making.
  26. Maintain confidentiality and security of patient information.
  27. Professional Practice
  28. Practice within the scope of nursing licensure and applicable Nurse Practice Act requirements.
  29. Adhere to the American Nurses Association (ANA) Scope and Standards of Practice and Code of Ethics.
  30. Demonstrate accountability, professionalism, and commitment to evidence-based nursing care.
  31. Maintain clinical competencies and participate in ongoing professional development.

Required Qualifications
  1. Graduate of an accredited nursing program.
  2. Current Registered Nurse (RN) license in the applicable state.
  3. Strong clinical assessment, critical thinking, and problem-solving skills.
  4. Experience in patient care, care coordination, and clinical decision-making.
  5. Excellent verbal and written communication skills.
  6. Ability to collaborate effectively with interdisciplinary healthcare teams.
  7. Strong organizational and time management skills.
  8. Ability to manage multiple priorities in a patient-centered environment.

Preferred Qualifications
  1. Previous experience in home health, long-term care, skilled nursing, residential care, or care management settings.
  2. Resident Assessment Instrument (RAI) process experience preferred but not required.
  3. Experience in leadership, supervisory, or charge nurse responsibilities.
  4. Familiarity with electronic medical records and healthcare technology platforms.
  5. Experience coordinating care across multiple disciplines and service providers.

Certifications (If Any)
  1. Current Registered Nurse (RN) License – Required
  2. Basic Life Support (BLS) Certification – Preferred or Required per facility policy
  3. Additional certifications in:
  4. Gerontology Nursing
  5. Care Management
  6. Home Health Nursing
  7. Long-Term Care Nursing
  8. Case Management are considered a plus.

Note- Travel Experience Required
