JOB SUMMARY
The Physical Therapist provides specialized physical therapy services within the home health and hospice environment, delivering comprehensive, patient-centered care while ensuring accuracy, safety, and compliance with all regulatory standards. This role provides rehabilitative services to home health and hospice patients, carrying out skills that support treatment, rehabilitation, safety, maintenance, and education for patients and their families, with particular attention to the geriatric population. Working as part of a collaborative interdisciplinary care team, the Physical Therapist plans and coordinates therapy services for individuals and families, assesses health needs and potential or existing hazards, and consults with or refers patients and families to appropriate agency or community resources as needed. Treatment and preventive procedures are delivered under medical direction, and care is evaluated, planned, implemented, supervised, and documented under the guidance of the supervisor or manager, in accordance with established standards of performance and organizational policies.
Key Responsibilities
• Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans.
• Supervise and coordinate care with Physical Therapist Assistants (PTAs).
• Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence.
• Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning.
• Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs.
Schedule / Shift
• 5 days/week
• 40 hours/week
Required Qualifications
• Licensed Physical Therapist in North Dakota.
• Current driver's license and available car for travel to patient homes.
Experience
• Strongly prefer 2 years previous experience as a Physical Therapist.
Skills
• Ability to provide rehabilitative services to home health and hospice patients.
• Skills supporting treatment, rehabilitation, safety, maintenance, and education for patients and their families.
• Assessment of health needs and potential or existing hazards.
• Consultation and referral to appropriate agency or community resources.
