JOB TITLE
LPN / LVN

JOB SUMMARY
Licensed Practical Nurses (LPN) / Licensed Vocational Nurses (LVN) provide basic nursing care to patients under the direction of a Registered Nurse (RN) or physician. They monitor patient health, administer medications, assist with treatments, and educate patients and their families.

Key Responsibilities
- Assess patient health and monitor vital signs.
- Administer medications and treatments as prescribed by physicians.
- Provide direct patient care, including wound dressing, catheter care, and personal hygiene assistance.
- Educate patients and families on health conditions, treatment plans, and preventative care.
- Collaborate with RNs, physicians, and other healthcare professionals to develop and implement care plans.
- Document patient observations, assessments, and interventions accurately and timely in the patient's chart.
- Respond to medical emergencies and provide basic life support.
- Maintain a safe and clean patient environment.

Schedule / Shift
Shift schedules will vary based on facility needs and may include days, evenings, nights, weekends, and holidays.

Required Qualifications
- Must possess a current, unrestricted LPN or LVN license in the state of practice.
- High school diploma or equivalent.
- Successful completion of an accredited LPN/LVN nursing program.
- Basic Life Support (BLS) certification.

Experience
- Prior experience as an LPN/LVN is often preferred, with varying facility requirements.

Skills
- Strong assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Proficiency in medication administration and patient care techniques.
- Ability to work independently and as part of a team.
- Detail-oriented with strong organizational skills.

EMR / Systems
Familiarity with electronic medical record (EMR) systems for charting and documentation is generally required.

Patient Population
Responsibilities may include providing care for diverse patient populations across various age groups, depending on the assigned unit or specialty.

Unit / Specialty
This role may be assigned to various units, including but not limited to Medical-Surgical, Geriatrics, Rehabilitation, or Long-Term Care settings.

Floating / On-Call
Floating to other units may be required based on departmental needs. On-call duties are typically not a standard requirement for this role but may be specified by individual facilities.
