JOB SUMMARY
Provide compassionate, high-quality nursing care to residents in a Long-Term Care unit, including medication administration, assessments, care planning, documentation, and coordination with physicians and the interdisciplinary care team.

Key Responsibilities
• Administer medications and treatments as prescribed.
• Perform comprehensive resident assessments.
• Develop and implement individualized care plans.
• Document resident care, progress, and observations accurately.
• Coordinate care with physicians and the interdisciplinary team.
• Educate residents and families on health conditions and treatment plans.
• Monitor residents for changes in condition and report to appropriate personnel.
• Maintain a safe and clean environment for residents.

Schedule / Shift
Ability to work the assigned schedule, including weekends and holidays as needed.

Required Qualifications
• Active Hawai?i LPN license
• Minimum of one year of recent Long-Term Care or skilled nursing experience
• Experience with electronic medical records

Certifications
• Current BLS certification
